Results Based Community Collaboration: Learnings from the Urgent Service Access Team (USAT) Initiative

Join in hearing how four organizations collaborate to support their inter-agency clients by developing coordinated care plans and care transition pathways. This has resulted in a 53% reduction in Emergency Department visits.

Together the USAT (Urgent Service Action Team) Project Management Team’s goal is to support USAT collaboration in working with clients and ensuring firm connection with primary health care, mental health and addiction services, and other resources that address the Social Determinants of Health.

In this session, USAT leaders will share the Team’s experience in celebration of Community Health and Wellbeing Week 2019 theme of Connected Teams, Connected People, Connected Communities.

Purpose of Webinar: Engage in knowledge about lessons learned by the Canadian Mental Health Association – Niagara Branch, Community Addiction Services of Niagara, Niagara Health, and Quest Community Health Centre as they collaboratively developed and supported the Urgent Service Access Team (USAT).
