This webinar will look at falls in the Long Term Care setting and more specifically residents who are experiencing consecutive falls.
Residents in Long Term Care Homes are often complex, living with multiple co- morbidities that increase their risk for falling, i.e. weak, frail, living with chronic health conditions, pain, cognitive impairment, dizziness, etc. The emphasis of this webinar will look at identifying contributing risk factors holistically both pre fall to which sets the foundation of fall prevention and then expand to post fall and root cause of the resident, their fall incidents in their environment.
This webinar will look at and discuss:
- On-going Resident Assessment
- Identifying individual fall risk factors.
- Care Planning and the development of action plans that outline specific interventions that can be implemented to reduce falls and/or injuries related to falls. With recognition that this is part of everything we do.
It will look at post fall processes including post fall huddles (things to discuss), the use of data to support this work, communication (what needs to be communicated and to whom) and documentation with the entire team.
